RCI Test Request Forms

For accurate identification of patients and specimens it is essential that request forms are completed fully, legibly and accurately. The provision of clinical details and the patient’s transfusion/obstetric history enables us to select the appropriate tests from the repertoire of services currently provided by the laboratory and accurately interpret the results obtained.

The BT345 test request form should be used for submitting patient samples for investigations of:

  • ABO/D Typing
  • Compatibility testing
  • Extended Phenotyping
  • Antibody Investigation
  • Antibody Quantitation
  • Antibody Titration
  • Investigation of AIHA + DAT
  • Transfusion Reaction Investigation
  • Other

The BT7 request form should be used for submitting patient samples for blood group and compatibly requests only. This form is only used in cases where the RCI laboratory functions as a Hospital Blood Bank for a facility.

Request forms are available on request from the RCI laboratory. The request form includes a sample pouch for safe transport of Biological Specimens.
